PUBLIC NOTICE
Muskogee County Emergency Medical Service (EMS) (Ambulance) will be destroying original paper copies of certain medical records that have already been converted into electronic media for electronic storage. This notice pertains to ambulance calls made up through December 31st of 2022.
All other medical records remain archived electronically as appropriate according to both state and federal requirements.
Patients desiring to obtain personal records should telephone the EMS business office at (918) 683-0130, at the Central EMS station, 200 Callahan Street in Muskogee, and make arrangements to retrieve the records. Completion of a Release of Medical Information form and photo ID will be required.
Records effected by this notice not picked up by August 6th, 2026 will be destroyed.
